Parents of Forest Hill 1st Graders
ALL Students entering First Grade are required to provide a Report of Health Examination for School Entry (PM171A). This form needs to be completed and signed by your health care professional after your child receives this mandatory first grade school entry health examination.
When must the health examination be completed?
A health examination must be completed any time between 18 months
before first grade entry through 90 days after entry into first grade.
In the interest of ensuring the health of all school children and promoting success at school, the State of California requires all students to present documentation of this physical within 90 days of entering first grade (Health and Safety Code §124085).
Health examinations must be given after February 19 2015 to qualify as a CHDP Examination.
